What To Bring To Your Appointment

As a new patient, we will need to collect some information from you when you arrive. We ask that you bring the following with you to your first appointment:

  • Your driver’s license or photo ID.
  • Your insurance card(s) if you have healthcare insurance.
  • A list of all medical conditions; diabetes, heart condition, etc.
  • A list of any prescription, over-the-counter medications you may be on.
  • Copay, Co-insurance, or Deductible if have
